Enrollment Information

CUSD On-line Student Enrollment

CUSD On-line Student Enrollment allows you to begin the enrollment process for students who live within the district boundaries.  You must have a valid address in the cities served by Capistrano Unified to use this system. For information on enrolling a student who  is currently attending and will be transferring schools within CUSD, contact the office at the appropriate school assigned to your residence to have your records transferred.  Please do not complete the Online Enrollment process.

Online Student Enrollment.

Please Note – Enrollment is different than the Registration Packet.

Appointments.

Schedule an enrollment appointment with the Registrar by calling or emailing the Registrar @ 949-589-6543 or  BRGoins@capousd.org.   The Registrar will require you to provide two documents as proof of residency, your child's immunization record, an original birth certificate, and transcripts from your child's previous school.  Additional documents may be required by your school site.  For further information on documentation, please see the section below.  The school site will deem the entire enrollment process to be complete and acceptable after you meet in person with the Registrar and provide the required documents.

Scheduled appointments may take approximately 15 minutes for enrollment and 15 minutes with the Academic Advisor. Walk-ins are welcomed, however may need to make additional arrangements to meet with an Academic Advisor if the Advisor is not available at the time of walk-in. Walk-in’s must have all necessary documents and be prepared to wait if necessary.  Students who currently have an individualized educational plan (IEP) or 504 plan will need to anticipate additional steps to enroll.

Class Selection.

The academic advisor will discuss classes and any other requirements with the student for class placement.

Enrollment Documents Required

 

Verification of Identity of parent, caregiver, licensed foster agency or group home representative, or California Superior Court-appointed legal guardian

  • A Driver’s License (any photo driver’s license  or CA ID Card is permitted) or,
  • A passport with photo ID
  • Government-issued document with photo ID
  • If an agent or representative of social services or foster care agency, appropriate agency identification
 

Verification of Residency inside CUSD attendance area of parent, caregiver, licensed foster agency or group home representative, or California Superior Court-appointed legal guardian For initial enrollment, parent or guardian must provide two documents from the list below: (P.O. Box Addresses are NOT accepted):

  • Utility bill (current electric, gas, or water/sew bill)
  • Current mortgage statement or rental agreement.
  • Rental agreement

  • Property Tax Bill

If the above-described documents are not available, the parent/legal guardian may offer other documents as proof of residence. However, since these documents may not be as reliable as those listed above, the District may also require additional verification, such as home visits and/or a
signed affidavit from the parent/legal guardian as to the validity of their residence. Examples of these documents are:

  • Bank statements
  • Moving company receipts
  • service letter from a utility company verifying residency
  • An escrow statement followed by verification of closing documents is also acceptable.

Proof of withdrawal from the previous school/district is required upon enrollment for all secondary school enrollments.

 

Proof of Age is required 

The following documents are acceptable documents for verification of the pupil’s age:

  • An official or certified copy of the birth Record,
  • A statement by the local registrar or county recorder certifying the date of birth,
  • A baptismal certificate of official hospital record of birth,
  • A passport, or
  • When none of the above is obtainable, an Affidavit for Proof of Age of Minor signed by the student's parent/legal guardian may be accepted. (Please note, this is not a District created form, this is an affidavit that would be provided to the District).

Proof of Immunization records

Students must meet all immunization requirements to be enrolled and attend classes. New students may be able to contact the prior school of attendance to obtain these records. Current CUSD students may request these records by clicking on the link to request the immunization record.

Immunization Requirements

 

In addition, CUSD requires tuberculosis screening for entry into preschool or before the first school entry. Therefore, if the student is enrolling from OUTSIDE the U.S.A. for the first time, a TB skin test will be required. Proof of written evidences must be provided with a Mantoux (PPD) Skin Test .  This test must be completed within the past eighteen months including results records in millimeters. If skin test results are positive, a x-ray date and statement that the child is free of communicable TB is required.

Emergency telephone numbers are required, including an out-of-state emergency number.

 

Other Documents and Information:

 

Transcript.

An official transcript is preferred at the time of enrollment; however unofficial transcripts or a report card from your previous school will be accepted. Prior school records will be requested by the school when your child enrolls.

 

For Special Education Students:

 

Individualized Education Plan/504 plan.

A copy of the student's most current of most recent IEP and Assessments or 504 plan must be provided at time of enrollment. Additional appointment will be necessary to complete enrollment

 

GATE Identified - (Gifted and Talented) Please provide a copy of the following:

  • Test scores from a Cognitive Abilities Test (e.g. NNAT, OLSAT CogAT). NOTE: Only assessments administered by a school are accepted - no private assessments. IQ tests are not accepted. 
  • Copy of most recent year's standardized test scores (e.g. SBAC/CASPP or PARCC, IOWA Basic Skills Test). If the most recent year is not available, the year prior may be accepted. Scores must be from the last two years.
  • Copy of the identifying school/district's GATE acceptance letter.

English Learners (EL)

  • a copy of the most recent CELDT scores or ELPAC scores if available.
 

Caregiver's Authorization.

A notarized signed Caregiver's Authorization Affidavit is required if another adult is assuming responsibility for the student with permission from the student's parents.

Registration

Immediately after enrollment, a Registration Packet may be provided to the student.  All required registration forms must be completed and turned in on the first date of attendance. 

All students must complete the registration forms every year. This includes current CUSD students who are automatically enrolled from the prior middle schools and SJHHS students from the prior school year of attendance. These required forms includes the following:

  • Parent Portal Data Confirmation Sheet 
  • McKinney-Vento Assistance Act
  • Migrant Education Program (if applicable)

5th Grade Students

5th grade students who attend Las Flores Elementary School, Tijeras Creek Elementary School, or Wagon Wheel Elementary School and are in the feeder pattern to attend Las Flores Middle School due to the attendance boundaries will NOT be required to enroll with the Registrar.  Records for these students will be automatically imported into the middle school.  The LFMS Academic Advisors will conduct the Spring course enrollment process in March to assist 5th grade students in selecting their elective classes for 6th grade.  All students will be required to attend summer registration.

School of Choice Process

The CUSD School of Choice allows families the opportunity to apply to attend another CUSD school that is not considered the neighborhood school serving the attendance area.

Families should review the information provided on the district CUSD website.  Any student approved on school of choice must consider that transportation is the responsibility of the parent/guardian.  School of Choice is only available during specific dates.    The School of Choice application period is open generally open early February  - early March each school year.  For further details regarding this program please check the Capistrano Unified School District website  here  for more information.

Parents/Guardians seeking placement for multiple children must submit a separate application for each child. 

Interdistrict Transfer Agreements

The interdistrict transfer agreement process applies to those parents who wish their children to attend school in a district other than that in which they reside. All interdistrict attendance agreement requests must be initiated in the district of residence.
